Position summary: The Occupational Therapist in the Geriatric Assessment Intervention Network (GAIN) Community Team is responsible for providing assessment utilizing various standardized tools with clinical judgment and developing interventions for older adults and seniors affected by emotional, psychological and physical impact of aging.
Responsibilities:1. Participate as geriatric assessor in inter-professional Comprehensive Geriatric Assessment (CGA)Completes clinical triage for new referralsData gathering and recording within all domains of Comprehensive Geriatric Assessment (CGA)2. Provides Clinical Care and Consultation Services to improve and/or maintain activities of daily living for frail older adults and seniors .
Analyzes frail older adults and seniors’ capabilities and expectations related to life activities through observation, interviews and formal assessmentsIn collaboration with the GAIN inter-professional team, establish care plans for patientsEducation:University Degree in Occupational TherapyRegistered and in good standing with the College of Occupational Therapists of OntarioRegistration with CAOT (Canadian Association of Occupational Therapists) or OSOT (Ontario Society of Occupational Therapists) an assetCurrent CPR and First Aid CertificationExperience & Skills :Two to three (2-3) years' working experience in the fieldDemonstrated extensive knowledge and skill working with individuals with various types of dementia, delirium, mental health, addictions, responsive behaviours, other neurological conditions, and those who experience an acute behavioural changeExperience and knowledge of working therapeutically with geriatric populations an assetDemonstrated sound knowledge of cognitive and mobility assessment toolsCertification in Gentle Persuasive Approaches, PIECES or Montessori preferredSkilled at crisis intervention and assessment preferredInitiative and adaptability skillsExcellent communication skills (written and verbal) in EnglishProficiency in other languages desired (Armenian, Greek, Mandarin, Tamil, Hindi, Urdu etc.)Others:Vulnerable Sector Check requiredA valid driver’s license and a car is required for home visitsProof of full COVID-19 Vaccination or Medical Exemption Certificate requiredSenior Persons Living Connected is a
