About the role:

The Sherbourne Health Bus has been serving homeless and underhoused clients in Downtown Toronto for over 30 years, making us a leader in the field of mobile health, with a model that has grown to include primary health care, brief case management and harm reduction.

We are presently looking for a Registered Nurse (RN) to join the Sherbourne Health Bus as an integral part of our mobile health team to deliver primary care, brief case management and harm reduction services to homeless and precariously housed individuals with complex mental health and substance use issues in the Downtown East Community. At Sherbourne Health, our Registered Nurses are key members of the interdisciplinary care team and work to their full scope of practice providing primary care and community health services across the lifespan.

Our nurses have taken leadership roles in exciting, multisectoral, partnership‑based community health projects, including community vaccination clinics (for influenza and COVID‑19), specialized HIV clinics for people without immigration status, low‑barrier COVID‑19 testing clinics, and community‑focused cancer screening initiatives. Similar opportunities will be available to the successful candidate for this role.

Day to day responsibilities:
  • Provide primary nursing care, in collaboration with the Health Bus Nurse Practitioner and social supports team, to homeless and underhoused individuals in downtown Toronto.
  • Conduct health assessments, focused health history taking and triage for episodic or urgent health concerns.
  • Implement therapeutic interventions for preventative health, health maintenance and health restoration that are suitable to clients facing homelessness and precarity.
  • Provide health education and health promotion that is responsive to client needs in areas such as preventative care, trans/non‑binary health, sexual health, substance use, harm reduction, and infectious and chronic disease.
  • Offer clinical support onsite to our low‑barrier drop‑ins for homeless and underhoused individuals, including WINK (Women in Need Clinic), STASH (harm reduction drop‑in), and our Trans Health Clinic.
  • Lead primary care appointments on the health bus that include wound care, footcare, venipuncture, STI testing and treatment, brief supportive mental health and crisis counselling, and episodic health assessments.
