About the role

As part of our MOBILE HEALTH BUS, the Nurse Practitioner (NP) will be an integral part of a team that brings primary care, brief case management and harm reduction services to homeless and precariously housed individuals with complex mental health and substance use issues in the Downtown East community.

Scope of the Mandate

The Nurse practitioner (NP) will provide a full scope of primary care and liaise as required with other members of the Health Bus team and external health care providers and community partners to ensure the delivery of comprehensive health care to clients on the Health Bus and complete follow up at Sherbourne Health and in the community as required.

The Nurse Practitioner understands the impact of the broad determinants of health on people’s ability to attend to their health care needs and provides care within a health promotion framework. The Nurse Practitioner will work collaboratively within a multidisciplinary team and in keeping with the core objectives and values of Sherbourne Health. This position is responsible for meeting and maintaining the standards of practice prescribed in legislation and regulations that apply to the position as outlined in the practice expectations specific to Registered Nurses in the Extended Class from the College of Nurses of Ontario.

In this role, the Nurse Practitioner works to full scope of practice.

Responsibilities and Tasks Client Service
  • Provides episodic primary care to clients, taking into account environmental, social, psychological and medical risk factors in a manner that can be understood by clients and is appropriate to their individual situation.
  • Discusses health concern/diagnosis with the client and together determines an appropriate plan of care.
  • Provides ongoing follow-up and evaluation of the plan of care with the client
  • Provides supportive counseling to clients with mental health/emotional concerns and refers to internal or external resources as necessary.
