About the Opportunity
We are partnering with a community-focused health organization in Scarborough to hire Nurse Practitioners for a variety of programs. These opportunities offer the chance to provide high-quality, patient-centered care across diverse populations, in a collaborative and supportive environment.

Key Responsibilities

Deliver comprehensive, family-centered primary health care within the full NP scope of practice.

Assess, diagnose, and manage acute and chronic conditions for clients across the lifespan.

Conduct client interviews, physical exams, and develop evidence-based treatment plans.

Administer immunizations according to Ontario’s publicly funded immunization schedule.

Maintain accurate and confidential client health records in compliance with CNO standards.

Collaborate with physicians and allied health professionals as part of an interdisciplinary team.

Participate in case conferences, peer reviews, and health promotion/outreach initiatives.

Support continuous quality improvement and best practice initiatives.

Qualifications

Masters Degree in Nursing with Primary Health Care Nurse Practitioner (NP-PHC) certification.

Current registration with the College of Nurses of Ontario (Extended Class) in good standing.

Minimum 3 years of NP experience in primary care, community health, or similar settings (CHC, FHT, NP-led clinic).

Current CPR (HCP) certification.

Strong assessment, clinical reasoning, and documentation skills.

Excellent interpersonal and communication skills; able to work collaboratively within a team.

Experience serving culturally, socially, and economically diverse populations.

Familiarity with EMR systems.

Valid Ontario driver’s license and access to a vehicle considered an asset.
