Mediflex Talent Solutions is seeking a compassionate and reliable Registered Practical Nurse (RPN) to support a long-term care environment in the Toronto area. This role is ideal for nurses who thrive in resident-focused settings, demonstrate strong clinical judgment, and work collaboratively with interdisciplinary teams.
You will provide hands-on nursing care, support PSWs, manage medication administration via eMAR systems, and ensure residents receive high-quality, dignified care.
Responsibilities- Provide hands-on nursing care to LTC residents
- Conduct resident assessments and update/document individualized care plans
- Administer medications using eMAR; follow all LTC medication protocols
- Assist with wound care, vitals, treatments, and ongoing resident monitoring
- Respond to emergencies promptly and support clinical decision-making
- Collaborate with physicians, RNs, PSWs, and allied health professionals
- Lead, support, and guide PSWs during the shift
- Maintain accurate and timely charting within PCC or other LTC EMR systems
- Communicate effectively with residents, families, and staff
- Adhere to all CNO standards, LTC legislation, and IPAC protocols
Requirements
- Active RPN license in good standing with the College of Nurses of Ontario (CNO)
- At least 6 months of experience in a long-term care or retirement home setting
- Legally eligible to work in Canada
- Current CPR/First Aid certification (BLS preferred)
- Valid Vulnerable Sector Check (or willingness to obtain)
- Fully vaccinated for COVID-19 + booster (or meets facility exemption criteria)
- Recent TB test / immunization record (or willingness to complete)
- Comfortable with medication administration using eMAR systems
- Experience working with residents who have dementia, Alzheimer’s, or responsive behaviours
- Gentle Persuasive Approaches (GPA) certification
- Experience supervising or supporting PSWs
- Experience in palliative or end-of-life care
