• Contributes to discharge and transition planning.

• Serves as the primary resource for organizing and coordinating patient care activities.
• Establishes and maintains respectful, collaborative, therapeutic and inter‑professional relationships.
• Coordinates and communicates patient care activities for a safe and seamless transition and appropriate transfer of accountability.
• Participates in hospital committees and other groups.

• Current registration with the College of Nurses of Ontario.
• Current BLS Certificate required.
• Recent Hemodialysis experience required; candidates without recent experience may be considered if they have completed at minimum Nephrology I or equivalent of the Nephrology Nursing Certificate and there are no qualified applicants.
• Nephrology Nursing Certificate preferred. Enrolled in Nephrology Nursing Certificate program upon hire with successful completion required within 3 years may be substituted if there are no qualified applicants.
• Experience with Windows 2000 and computer order entry an asset.
• Knowledge of Baxter dialysis equipment an advantage.
• Excellent assessment skills required.
• Excellent communication skills, both oral and written.
• Satisfactory attendance record.

Must be available for all three shifts, 8‑ and 12‑hour shifts (days/evenings) including weekends and holidays at the main hospital site and off‑site satellite unit. On call as required.
