Heart failure is a complex clinical syndrome that benefits from early diagnosis and evidence‑based management. The Royal Cornwall Hospital Heart Function Nursing Service provides clinical management advice for patients admitted acutely with a primary diagnosis of left ventricular systolic dysfunction (LVSD). The service fully participates in the National Heart Failure Audit and delivers:
- 52‑week a year in‑patient cover for patients admitted with a primary diagnosis of heart failure
- Education and advice on pharmacological, non‑pharmacological and complex device management to patients and MDT members
- Diagnostic, two‑ and six‑week referral to review rapid access heart‑function clinic (NICE pathway)
- Out‑patient elective DC cardioversion and associated follow‑up
- Follow‑up clinics for complex or high‑risk heart‑failure patients requiring close monitoring or medication optimisation
- Provide clinical management advice and education to patients admitted with LVSD and related conditions
- Deliver pharmacological, non‑pharmacological and device‑management advice in line with best practice guidelines
- Coordinate referrals to rapid‑access heart‑function clinics and attend assessments
- Conduct out‑patient DC cardioversion procedures and manage post‑procedure care
- Run follow‑up clinics for high‑risk patients and optimise medication regimens
- Participate in audit and research activities related to heart failure care
- Deliver presentations and “priorities of care” scenarios during interviews where requested
- NMC registration
- Non‑medical prescribing qualification or commitment to undertake
- Completion or commitment to undertake extended‑role scope competencies such as DC cardioversion (including ALS)
- Master’s level education or equivalent in a relevant discipline
- University qualification in the specialty
- Teaching, assessment or mentoring qualifications
- Managerial experience or experience leading a team
- Recent relevant nursing experience within the specialty
- Audit or research experience
- Teaching and education experience
- Experience caring for patients with a heart‑failure diagnosis
- Identity checks
- Professional registration and qualification checks
- Employment history and reference checks
- Occupational Health clearance
- Satisfactory Disclosure and Barring Service check
