We are seeking a compassionate and motivated Registered Nurse Associate (RNA) to join our community nursing team, delivering high-quality care to patients in their own homes. This role is ideal for a nurse who is passionate about person-centred care and supporting individuals to remain independent within their community.

You will work as part of a multidisciplinary team, carrying out planned home visits, supporting Registered Nurses, and contributing to the ongoing assessment and care of patients with a range of health needs.

Deliver safe, effective, and compassionate nursing care during home visits
Support patients with long-term conditions, post-discharge care, and wound management

Undertake clinical tasks in line with your scope of practice and training

Monitor, record, and report patient conditions and escalate concerns appropriately

Maintain accurate and timely clinical documentation

Work collaboratively with Registered Nurses, GPs, and other healthcare professionals

Promote patient dignity, independence, and wellbeing

About You
Registered Nursing Associate (RNA) with current NMC registration

Experience in community, primary care, or home visiting is desirable (but not essential)

Strong communication and interpersonal skills

Ability to work independently and manage a community-based caseload

Compassionate, reliable, and committed to high standards of care
