- Key Responsibilities

As a Practice Nurse, your responsibilities will include:

Managing chronic diseases through assessment, diagnosis, and developing personalized care plans
Delivering QOF and enhanced service requirements efficiently
Conducting investigations such as spirometry and reversibility testing
Providing wound care, including complex wound management
Performing health checks, NHS health checks, and cardiovascular assessments
Monitoring blood pressure, blood glucose, and conducting ECGs and phlebotomy
Offering health promotion services like cervical screening, travel immunizations, and smoking cessation support
Supporting mental health initiatives and medication monitoring
Additional clinical duties such as ear care and minor surgery support
Conducting safeguarding assessments and social risk evaluations

Leadership & Management

Lead and support the nursing team through supervision, appraisals, and team development
Contribute to workforce planning and service development initiatives

Candidate Requirements

Current NMC registration as a Registered Nurse
Previous general practice experience preferred
Excellent communication and interpersonal skills
Ability to work both independently and within a team
Strong organisational skills with a patient-centred approach
Knowledge of confidentiality and information governance
Competent in IT systems; experience with Systm One is desirable
