The Montefiore Licensed Practice Nurse contributes standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing's Social Policy Statement. The Licensed Practical Nurse actualizes the vision, mission, values, balanced score card performance measures for Montefiore Medical Center. The LPN participates in performance improvement/research activities; maintains professional practice standards and clinical expertise;
and demonstrates leadership skills. The LPN demonstrates core and unit specific competencies based on the patient population served. The Licensed Practical Nurse contributes to the actualization of the vision, mission, values and philosophy and balanced score card performance measures for the Medical Center. He/she is committed to maintaining the standards of the practice of nursing and is responsible for the delivery of care to the neonate, pediatric, adolescent, adult and/or elderly patient within their scope of practice.
He/she participates in performance improvement, research activities and maintains clinical expertise.
- Provide patient/family teaching and discharge instructions.
- Initiates measures for patient support.
- Identifies and reports needs in 100% of assigned patients.
- Implements content and teaching methodologies for cognitive, functional, maturation levels identified by the RN.
- Facilitates discharge in collaboration with RN.
- Able to set priorities, problem‑solve, and use proper judgment in difficult situations and be flexible in non‑emergent situations.
- Takes action within scope of practice to optimize achievement of patient outcomes.
- Differentiates between priority and non‑priority work.
- Completes assignment within established time frames.
- Escalates patient issues following established communication systems.
- Participates in identification, investigation and resolution of actual/potential risk issues.
- Adapts to change in the work environment, manages competing demands, changes approach or method to best fit the situation.
- Provides and maintains safe environment for patients/families through preventive and interventional measures.
- Administers medications with 100% compliance within scope of practice and MMC policies and procedures.
- 100% compliant with institutional P&P/Regulatory Agency Guidelines.
- Adheres to MMC's infection control, safety & disaster policies and procedures.
- Orients 100% assigned patients/families to environment ensuring comprehension of information presented.
- Participates and responds to investigation and resolution of actual and/or potential risk issues.
- Uses equipment in accordance with MMC policies and procedures.
- Documents direct patient care as per MMC policies and procedures.
- Records medication administration, specimen collection and other assigned tasks as per MMC policies and procedures with 100% accuracy.
- Records nursing interventions and observation of patient response according to MMC policies and scope of practice accurately and timely.
- 100% completion of assigned patient admission/advance directives/discharge documentation as per MMC policies and procedures.
- Participates, practices and promotes continuous quality improvement.
- Contributes to problem identification and problem solving in the work environment.
- Recognizes responsibility extends beyond one's own patient population; supports and assists co‑workers.
- Participates in collection and documentation of data relevant to planning/evaluation of patient care.
- Participates on work groups or data collection annually.
- Demonstrates accuracy and thoroughness in work.
- Communicates effectively with patients/families and members of the health care team.
