A frontline outreach and through-care role supporting people who are rough sleeping or at risk of rough sleeping to access services, stabilise accommodation, and move towards sustained independence.

THE ROLE
As a Rough Sleeping Navigator, you will work as part of a multi-agency Rough Sleeping Reduction Service, providing proactive, person-centred support to individuals entrenched in or at risk of rough sleeping, with the aim of preventing homelessness and promoting longer-term stability.
Manage a caseload of clients who are rough sleeping or at risk of rough sleeping, delivering tailored support plans
Conduct street outreach, accommodation visits, and engagement across Slough and occasionally outside the borough
Work closely with Outreach, Housing Demand and Temporary Accommodation teams to identify and support at-risk clients
Complete needs and risk assessments and coordinate multi-agency interventions
Support clients to access health services, substance misuse support, housing, benefits, training and employment opportunities
Provide cover for outreach sessions, including early mornings, evenings and occasional night-time outreach
Maintain accurate case records, reports and administrative documentation in line with service requirements

THE CANDIDATE
The successful candidate will have previous experience working in a rough sleeping, homelessness or outreach-based role and be confident supporting vulnerable individuals with complex needs.
Experience delivering frontline homelessness or rough sleeping support
Strong understanding of multi-agency working and local authority pathways
Ability to assess risk, manage complex caseloads and develop person-centred support plans
Confident engaging clients in a variety of settings, including street-based outreach
Strong record keeping, communication and safeguarding awareness

A DBS check is required for this role. THE CONTRACT

WORKING HOURS:

Monday – Friday, 09:00 – 17:00
LENGTH OF

CONTRACT:

3 Month Contract – Temporary
RATE: £20.00 per hour LTD / £17.162 per hour PAYE (inclusive of holiday)
