Reporting to the Patient Care Manager or Resident Care Manager and working in collaboration with the interdisciplinary team, the Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/residents according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. The LPN functions as an independent practitioner, while recognizing situations such as increased patient acuity or care that falls outside the scope of an LPN as appropriate for accessing the RN/RPN for collaborative support and/or clinical intervention.

• Assists patients/residents in meeting physical, social, spiritual, and psychological needs
• Consistent with mission, vision, and values of Providence Health Care (PHC)
• Participates in care planning, assessments, interventions, and evaluations
• Documents information and advocates for patients/residents
• Attends various meetings including care conferences
• Assists with orientation of new staff and students
• Responds to and reports incidents following established procedures
• Performs clerical duties as required

Education:

• Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience
• Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
• Current basic cardiac life support certification

Skills & Abilities:

• Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
• Physical ability to carry out the duties of the position
• Ability to apply decision making and analytical skills
• Ability to role model conflict resolution strategies
• Ability to prioritize and organize work
• Ability to teach and identify patient/resident learning needs

• Program and Scope: Holy Family Hospital Rehabilitation Unit (HFH Rehab) is one of the largest rehabilitation referral centres in BC. The unit has 65 inpatient beds dedicated to supporting the specialized rehabilitation needs of older adults following a life-altering event, such as a stroke, other neurological issues, orthopedic trauma, as well as major surgeries like hip replacements and leg amputations. In addition, more than 15,000 patients visit HFH Rehab each year to receive outpatient rehabilitation services for needs resulting from illness or injury
