About Vancouver Long Term Care

Vancouver Coastal Health is looking for a Nurse Practitioner to join the Professional Practice team focused on Long Term Care facilities working out of Vancouver, BC. This presents a great opportunity to provide care as Most Responsible Provider to residents of facilities in this dynamic city with a rich cultural background. You will practice independently, with a team on the ground in each facility as well as within a larger group of Nurse Practitioners who provide care to long term care facilities across the lower mainland.

This is a Post-Graduate Advanced Education (PGAE) program opportunity—a six-month initiative designed to attract, train, inspire, and retain outstanding Nurse Practitioners who provide exemplary care to a diverse patient population. The program is completed part time alongside clinical work. The program features structured onboarding with supervised clinical training in elderly care. Didactic sessions are delivered in an interdisciplinary setting and focus on core competencies in elderly, clinical reasoning, simulation, work-life balance, and quality improvement.

Throughout the program, NP participants will receive regular follow-up and dedicated support from an NP Lead to ensure a smooth and successful transition into their role. A minimum 18-month return-of-service commitment is required upon completion of the PGAE program. Participants are expected to demonstrate a strong commitment to their clinical position and to the specialty, with the intention of contributing as subject matter experts to the growth and sustainability of future PGAE programs.

As a Nurse Practitioner with VCH you will:

  • Be responsible and accountable for the comprehensive assessment of patients and clients including diagnosing diseases, disorders and conditions.
  • Initiate treatment including health care management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits and conditions, and employer policies and procedures.
  • Provides professional guidance to other health professionals and participates autonomously as a member of the interdisciplinary health teams.
  • Collaborate with clients and other health professionals to identify and assess trends and patterns that have implications for clients, families and communities; develops and implements population and evidence-based strategies to improve health and participate in policy-making and quality improvement activities that influence health services and practices.
  • Participate in peer, program and self-review to evaluate the outcome of the services at the client, community and population level.
  • Be responsible for own professional development and maintaining competence in the position, and maintaining privileges as active medical staff as required.
  • Report to the Senior Director, Nurse Practitioners, and with accountability to the Medical Director for the service area.

    Education & Experience

  • Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
  • Prior experience as a Nurse Practitioner in the field an asset, but open to all motivated applicants.

  • Ability to perform the full scope of NP duties and responsibilities.
  • Applies for and maintains core and non-core privileges, as required by the program, as active medical staff within the VCH Regional Department of Nurse Practitioners and cross-appointments as described in the VCH Medical Staff Rules and Medical Bylaws.
  • Ability to apply knowledge of developmental stages, behavioral sciences, pathophysiology, psychopathology, epidemiology, and evidence-based practice to clinical diagnosis and patient management.
