We are recruiting for an Advanced Clinical Practitioner to support generalpractice in Walsall and in particular, the PCN and its objectives. The successful candidate will work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiatingtreatment as appropriate. The post includes working within the Walsallpractices.
The post will work with all the 5 clusters of GP surgeries known as Primary Care Networks across the North, East, South or West of Walsall. The post willbe based at and provide the service from the GP surgeries, with somerequirement to attend team meetings and training at Our Nets Head Office,located in Bloxwich, as well as training at other local or regional venues as necessary.
ACPs:
Candidates will have a masters degree. Is working at a masters levelaware or equivalent that encompasses the four pillars of clinical practice,leadership and management, education and research, with demonstration of corecapabilities and area specific clinical competencies. Has qualified from an approved 18-month training pathway or equivalent and qualified as an independent prescriber
Main duties of the jobTo work as an autonomous practitionerworking independently and with other health care professionals to assess,diagnose and treat patients with a range of acute, non-acute and chronicmedical conditions, initiating treatment as appropriate.
To assess, diagnose and treatundiagnosed and diagnosed conditions for patients within a primary care setting.
To develop innovative concepts, models,methods and practices which deliver new and improved primary care services tomeet the changing needs of primary care.
To trial and adopt new working methodsdesigned to manage the increasing demand in primary care.
To provide education and training toother staff and students as appropriate and to take a lead role in encouragingteamwork and sharing best practice.
To undertake audit and research if andwhen required.
To promote healthy lifestyles, health promotion and disease prevention, by suggesting and recommending possible solutions.
Manage caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence

Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time eg. Pathologyresults, common/minor ailments, acute conditions, long term condition reviews,etc.

Our Net Health Services is a provider of primary care service to NHS patients in Walsall. Established in 2019 with the support of GPs in Walsall, we aim to deliver excellent care to patients when they need it most.

Be aware of and adhere at all times to Our Net Health Services Ltdspolicies and procedures, including the organisations equal opportunitiespolicies and with due regard for health and safety, and the relevant policies and procedures of the host Primary Care Network.
Work flexibly and be available for occasional out-of-hours working as appropriate.
Work with the general practice team to ensurethe practice is compliant with CQC standards where medicines are involved.

- Reguistered Nurse (NMC) or Allied Health Professional with current registration (HCPC)
