Community Specialist Palliative Care Nurse

Band 6 Community Specialist Palliative Care Nurse – South Team. Base location:
Whitnash Lodge. 37.5 hours weekly. We are looking for an enthusiastic and motivated nurse with a passion for palliative care, who has recent study or training to support their knowledge and skills, and who is comfortable working autonomously and collaboratively within a multidisciplinary team. The post holder will demonstrate excellent communication skills in supporting patients and their families and will be committed to providing high quality compassionate care.

• Actively manage a clinical caseload of patients requiring specialist palliative care.
• Provide specialist advice, education and training to the team and wider workforce.
• Lead and deliver changes and initiatives in management of specialist palliative care services across Warwickshire.
• Work collaboratively with the locality team to implement the specialist palliative care element of the SWFT Business plan.
• Provide specialist palliative care support across Warwickshire to meet service needs.
• Deliver teaching and education programmes.

• Registration as a Nurse.
• Experience in palliative or end‑of‑life care.
• Excellent communication and teaching skills.
• Ability to work independently and as part of a multidisciplinary team.
