Join our team as an Advanced Nurse Practitioner.

Work within our practices in the Wellingborough neighbourhood to deliver high-quality, same-day care to our patients.

Were looking for an experienced Advanced Nurse Practitioner to join our Neighbourhood Access Service, part of the innovative New Models Programme.

You'll work autonomously and as part of a multidisciplinary team within primary care to provide expert assessment, diagnosis, treatment, and evaluation for patients presenting with minor illness, acute conditions, and common general practice presentations, including women's health.

Main duties of the job

As aNurse Practitioner you will provide autonomous care for our patients presenting with minor/acute illness, and common general practice presentations including those relating to womens health. You will demonstrate safe clinical decision making and provide expert care by undertaking initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

Working with GPs and nursing team in the Practice you will continually review and evaluate services and work with the management team in implantation of change.

Role Summary

As a Nurse Practitioner you will provide autonomous care for our patients presenting with minor/acute illness, and common general practice presentations including those relating to womens health. Overview of Responsibilities

Clinical

  • Provide clinical care to our patients. This will include reducing the demand on the GP time by the assessment and manage of patients and in the community by means of telephone triage and clinic appointments.
  • Use advanced skills and expert knowledge to assess the physical and psycho social needs of patients with common primary care conditions and minor illness, instigating and providing in partnership with other care providers, therapeutic treatments based on best available evidence in order to improve health outcomes.
  • Use advanced skills and expert knowledge to make comprehensive and focussed assessments, independently and proactively managing care of the patients.
  • Promote patient independence and self-care where appropriate.
  • Undertake the role of independent non-medical prescriber in the treatment and care of patients
  • Review pathology results to ensure appropriate treatment and care
