Rate: 31.97 per hour

Hybrid
Driving licence + own vehicle required

Telford & Wrekin Council are seeking an experienced Senior Social Worker to join their Early Intervention and Prevention Team. This is a fantastic opportunity to support adults in the community by providing proactive, strengths-based interventions that promote independence and prevent escalation of need.

Key Responsibilities:

Carry out Care Act assessments, reviews, and person-centred planning

Work collaboratively with health and community partners

Provide professional guidance to colleagues within the team

Support individuals to achieve positive outcomes through early intervention

Manage a reduced, well-structured caseload

Requirements:

SWE registration

Strong experience within Adult Social Care

Driving licence and access to a car (essential)

Ability to work autonomously in a hybrid model
