Per Diem Social Worker needed for the Boston Children’s Hospital Primary Care Center in Weymouth – free parking!

• Providing clinical social work services to assist patients and families with personal, family, medical and environmental difficulties.
• Performing psychosocial and risk assessments, assigning diagnoses, and developing and implementing interventions and treatment plans.
• Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan
• Implementing and following up on treatment and provision of needed services.

• Education:
  
  Master’s degree in social work
• Experience:
  
  Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills. Experience with social services agencies, understanding of social determinants of health and trauma ideal. Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus!
