Community Mental Health Nurse (Older Adults)

We are looking for an enthusiastic Community Mental Health Nurse to join our Older Adult Team within Caithness.
Role overviewYour role will be assisting in managing the clinical care of patients with mental ill health as delegated by Senior Practitioners (CPN) with adults over the age of 65 or of any age if a memory concern. It will involve the assessment; care planning, implementation and evaluation of the care plan, in partnership with the client, family and carers.
The role will involve medication monitoring, liaising with psychiatrists, Primary Care and other Social Care services, and referring on to other services as appropriate. You will be an active member of an MDT. You will also be supervising students and Support Workers on occasion.
Key responsibilities- Assist in managing the clinical care of patients with mental ill health.
- Conduct assessments and develop care plans.
- Implement and evaluate care plans in partnership with clients, families and carers.
- Monitor medication.
- Liaise with psychiatrists, primary care and other social care services.
- Refer to other services as appropriate.
- Act as a member of an MDT.
- Supervise students and Support Workers on occasion.
- Registered with the NMC and work within the code of conduct at all times.
- Valid driving licence; your own vehicle may be required for work purposes.
- Full‑time permanent post of 37 hours as per service requirements.
