Job Summary
Join the Care Coordination team at the Royal Wolverhampton NHS Trust, working remotely to triage patients across the Wolverhampton locality. The role involves supporting patients with acute needs to avoid admission, coordinating with the Urgent Community Response Team and West Midlands Ambulance Service, and providing ongoing support for patients with long‑term conditions from home. In addition to remote triage, a junior sister/charge nurse will lead the team, mentor staff, and drive service innovation.
Responsibilities- Triage patients by telephone or video consultation to direct them to appropriate services.
- Collaborate with the Urgent Community Response Team and West Midlands Ambulance Service to deliver safe community care.
- Support patients with long‑term conditions receiving therapy and health support at home.
- Provide a face‑to‑face triage assessment for cases that would otherwise be referred to a District Nurse or Therapist.
- Offer managerial leadership support, staff development and lead service innovation initiatives.
- Utilise computers and web‑based applications confidently; maintain accurate records and data entry.
- Valid driving licence and access to a car.
- National Nursing and Midwifery Council (NMC) registration number.
- Leadership experience or qualification.
- Proficiency in databases, standard keyboard skills, and knowledge of IT packages such as Word, PAS, and e‑rostering.
- Post‑registration course for care settings (e.g., Health Assessment Module).
- Evidence of working within Community Nursing.
- Experience in Rapid Response/Emergency Care Triage.
