Job SummaryProvides speech language pathology services in accordance with the College of Health and Care Professionals of BC standards of practice, Fraser Health mission, values and speech language pathology acute care standard, and in collaboration with the interprofessional care team to provide patient- and family-centered care. Provides assessment, diagnosis and prognosis of speech language and dysphagia, treatment planning and intervention development, implementation and evaluation, and patient consultation and education to enable the patient to achieve optimal health outcomes in communications and dysphagia function.
Participates in approved speech language pathology and interprofessional research, special projects, and program quality and safety activities.
- Provides speech language assessment of the patient's communication, speech and swallow functional abilities, needs and goals using records review, information gathering, patient interviews, and evidence-based assessment methods; collaborates with the health care team and monitors the patient’s health to support speech, language, voice and swallowing performance.
- Determines diagnosis and prognosis and develops an evidence-based, outcome-focused intervention strategy; discusses analysis with patient and caregivers, sets short- and long-term goals, and selects appropriate intervention models.
- Implements interventions according to established guidelines and educates patients and caregivers on self-management, health promotion and prevention.
- Participates in interprofessional care planning, updates care plans, and coordinates care during transfer or discharge as appropriate.
- Evaluates and documents the effectiveness of interventions through ongoing assessment, outcome measurement, and modification of plans to optimize functional independence and communication/swallowing outcomes.
- Facilitates appropriate referrals for speech language pathology services and coordinates with other health care and community programs.
- Collaborates with patients and families in developing and modifying the care plan; maintains therapeutic relationships with consideration for privacy, dignity and patient autonomy in decision making.
- Maintains patient records in approved formats and documents progress, plan changes, and test/treatment results; compiles statistics as requested.
- Delegates components of treatment to speech and hearing assistants with clear direction and documentation of transfer of function to ensure quality care.
- Acts as a resource to other clinicians, provides information and in-services to the health care team, and represents speech language pathology in meetings as needed.
- Participates in team and program activities to enhance care delivery and promote evidence-based practice; identifies areas for improvement and provides recommendations.
- Performs other related duties as assigned.
- Current registration with the College of Health and Care Professionals of BC including applicable College certification(s).
- Knowledge of speech language pathology theory and practice within a patient- and family-centered model of care.
- Knowledge of CSHBC standards of practice and clinical guidelines.
- Knowledge of communication and dysphagia standardized tests and related equipment and techniques.
- Broad knowledge of other health care professions and their roles in patient care.
- Effective communication with patients, caregivers, team members, and other health professionals in written and verbal form.
- Ability to teach and provide effective consultation to patients, caregivers, and other health professionals.
- Ability to work independently and collaboratively; prioritize workload and adapt to unexpected events.
- Basic knowledge of research processes and methodology.
- Ability to operate related equipment and software applications.
- Physical ability to perform the duties of the position.
