We have the perfect Relief Full-Time opportunity for a Clinical Pharmacist to join our team at Abbotsford Regional Hospital and Mission Memorial located in Abbotsford and Mission, BC. This position is available until approximately September 2027 or until the return of the incumbent.

Clinical Pharmacist Services
  • Individualized pharmacotherapy

Detailed Overview

The Clinical Pharmacist provides clinical pharmacy services including individualization of patient medication therapy; patient medication counselling; consultation with physicians, nurses and other health care providers to develop and discuss therapeutic plans; assistance in development of drug therapy guidelines for individual disease conditions. Provides support to the Pharmacy & Therapeutics Committee by preparing written summaries and recommendations on drug usage; contributes items such as case studies and articles for submission to professional journals;

acts as department resource person in the assigned area of responsibility; participates in medication use evaluations and clinical drug trials; provides instruction and/or supervision to pharmacy students and residents.

Additionally, the Clinical Pharmacist performs drug distribution duties including processing prescriptions by reviewing prescribed medication therapy; validating prescriptions; identifying safety concerns; contacting the subscriber and/or the health care team to resolve discrepancies or problems; documenting discussions or modifications to the medication therapy and dispensing medications.

Responsibilities
  • Consult with physicians, nurses and other health care providers to develop and discuss therapeutic plans appropriate for each patient based on chart review and assessment of clinical responses. Ensures documentation is maintained in the patient health record.
  • Individualize patient medication therapy by reviewing patient charts, laboratory results, meeting with patient and health care team to assess clinical response, and modifying therapeutic plans.
  • Provide patient medication counselling, summarizing information and ensuring understanding.
  • Assist the health care team in developing drug therapy guidelines for individual diseases based on current literature.
  • Participate in ward rounds, program team meetings and provision of pharmacy services at unit level.
  • Act as department resource person in the assigned area of responsibility.
  • Contribute case studies and review articles to professional journals.
  • Provide instruction and supervision to pharmacy students and residents.
  • Participate in quality improvement activities.
  • Perform drug distribution duties, including interpreting prescriptions and dispensing using computerized system.
  • Review prescriptions for therapeutic suitability.
  • Communicate with physicians, patients and other health care team members for clarification and problem resolution, documenting outcomes.
  • Ensure accurate dispensing and labeling; maintain dispensing records.
  • Participate in data collection for medication use evaluation projects and maintain inventories.
  • Participate in clinical drug trials and documentation.
  • Implement medication safety initiatives and quality control activities.
  • Attend staff meetings, committees and other programs as required.
