Position: Vision Loss Client Navigator Role
Join Vision Loss Rehabilitation Canada as a Vision Loss Client Navigator, making a difference for individuals living with vision loss. This full-time role focuses on client support and healthcare coordination.

In this permanent position, you will serve as a key contact for clients and manage their integration into healthcare services after a vision loss. Your responsibilities will include assessing clients' needs, ensuring timely access to resources, and facilitating communication between service providers. Support clients as they regain independence.

Key Responsibilities:

• Organize and schedule therapy appointments with service providers

• Solicit client feedback to enhance service quality

• Provide proactive follow-ups to maintain client connections

• Explore new care options tailored to client needs

• Ensure documentation meets VLRC standards

Requirements:

• Proven experience in client service within health sectors

• Relevant diploma in Social Services or similar

• Strong interpersonal and communication skills

• Understanding of client-centered care methodologies

• Experience with vision loss support systems
