Community Nurse - Licensed Practical Nurse

Fraser Health is looking for a dedicated Community Health Nurse to independently manage assigned clients. In this role, you will deliver comprehensive assessments, interventions, and health education tailored to complex chronic conditions. Working with an interprofessional team, you will facilitate care plans and help clients transition smoothly across the healthcare system.

Key Responsibilities:

• Build therapeutic relationships with clients

• Assess health needs and coordinate care plans

• Deliver direct nursing care and explain health interventions

• Promote health education and self-management

• Conduct joint home visits with healthcare professionals

Requirements:

• Current practice as a Licensed Practical Nurse in BC

• One year of relevant direct care experience

• Valid BC Driver's license is required

• Demonstration of excellent communication skills

• Ability to manage multiple client healthcare issues
