Position: Registered Community Health Nurse Role
Enhance community health as a Registered Nurse with Fraser Health, focusing on chronic disease management and client education. This position fosters a collaborative approach to care delivery.

You will serve as part of an interdisciplinary team, addressing the needs of clients within their community. Key Responsibilities:

• Develop trusting relationships with diverse clients

• Conduct thorough assessments and care coordination

• Facilitate health promotion and wellness initiatives

• Document interventions and client progress accurately

• Support transitions between care services and providers

Requirements:

• Registered Nurse status with BCCNM

• Minimum of one year in community or outpatient settings

• Required BC Driver's license for business travel

• Proven ability to communicate effectively with clients

• Knowledge of community resources and healthcare services
