Detailed Overview
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and Fraser Health's mission and values, the nurse works collaboratively as part of an inter‑professional team to provide direct patient care to clients of the Urgent Primary Care Centre (UPCC) or Primary Care Clinic. They provide initial client consultation, assessment, coaching, health promotion, intervention, and follow‑up based on client needs and urgency, ensuring clients, families, and caregivers have timely, person‑centred, continuous, and culturally safe quality health services.

The nurse facilitates and manages client transitions across the health care continuum, collaborating with acute, primary, and community care providers to optimize recovery and minimize avoidable admissions. The role promotes a trauma‑informed and evidence‑based approach, establishes dignifying relationships, encourages knowledge exchange, and empowers clients to engage in community health. They also support navigation of the health care system for clients and families.

Responsibilities

Provides direct nursing care to clients receiving scheduled or urgent primary care; assesses clients and collects information from various sources using observation, communication, data review, and physical assessment; makes nursing diagnoses and determines appropriate interventions.

Processes physician or nurse practitioner orders by initiating unit standing orders, administering medications, carrying out treatments, and monitoring and documenting effects.

Establishes a therapeutic relationship with clients—including in person and over the telephone—to ensure client choice, autonomy, dignity, and privacy.

Screens referrals, provides individualized assessments, inter‑professional care planning, and interventions; assists clients with complex conditions and coordinates care with primary providers and community resources.

Develops a comprehensive shared care plan with the inter‑professional team, primary provider, client, and family, and supports transition of the plan to community providers.

Provides direct planned and unplanned client care, identifies additional services per guidelines, and explains care to clients and families.

Reviews client care plans with the team to determine timing and referral to other services; initiates, monitors, and evaluates the effectiveness of short‑ and long‑term plans to meet client goals.

Discusses clinical findings with clients/families, collaborates to develop action plans and sets goals directed at clinical needs and self‑management; accesses system information and resources for comprehensive care planning.

Supports clients and families during clinic visits, provides decision‑making information, advocates for their needs, and delivers care during transitions.

Arranges and participates in outreach nursing care at home or suitable settings; conducts assessments, determines status, evaluates medication response, and provides advice on community resources.

Provides health counseling including education, self‑management, self‑monitoring, and wellness promotion through clinic, telephone, or home/outreach visits.

Maintains clinical records—intake screens, assessments, interventions, treatment formulations, care plans, and progress notes—and records statistical information as required.

Maintains professional practice growth by reviewing literature, attending workshops, and networking with health professionals.

Participates in departmental quality improvement, risk management, and research activities as needed.

Participates in orientation and ongoing education of nursing staff and students as a preceptor.
