Key Responsibilities:

• Assess patients and develop individualized care plans

• Collaborate with families and care providers in care planning

Requirements:

• Current registration with BCCNM as a Registered Nurse

• Two years of recent acute care experience with geriatrics

• Strong communication skills for diverse patient interactions

• Ability to coordinate and prioritize nursing care

• Knowledge of community resources for patient referrals
