Community Health Nurse

Position: Fraser Health Community Health Nurse

Key Responsibilities:

• Screen referrals and provide tailored client assessments

• Collaborate on comprehensive care planning

• Deliver direct nursing care based on clinical guidelines

• Monitor care plans to ensure client needs are met

• Arrange joint visits with other health professionals

Requirements:

• Registered Nurse with current BCCNM registration

• At least one year of relevant clinical experience

• Valid driver's license and personal vehicle

• Strong communication and assessment skills

• Ability to manage complex health situations
