Key Responsibilities:

• Develop personalized care plans for Indigenous clients

• Assess and monitor physical, psychological, and spiritual needs

• Coordinate with community services for best outcomes

• Deliver health education and promote client self-management

• Maintain accurate records and documentation as per standards

Requirements:

• Valid LPN registration with BCCNM

• Recent nursing experience focused on Indigenous health

• Knowledge of community resources for Indigenous clients

• BC Driver's license and access to a vehicle

• Ability to communicate effectively and compassionately
