This position requires a Licensed Practical Nurse with experience in geriatric care, emphasizing health education and collaboration with healthcare providers. In your role, you'll coordinate care, implement health strategies, and assist clients in navigating the healthcare continuum.

Key Responsibilities:

• Establish therapeutic relationships with clients and families

• Conduct client assessments and develop care plans

• Provide direct nursing care and health education

• Collaborate with healthcare teams and community resources

• Maintain accurate clinical records and documentation

Requirements:

• Valid Licensed Practical Nurse registration with BCCNM

• One year’s clinical experience in chronic condition management

• Strong communication and problem-solving skills

• Ability to work independently and as part of a team

• Valid BC Driver's license and access to a vehicle
