In this dynamic role, you will support clients referred through Community Living British Columbia, focusing on their successful integration into the community. You will create tailored goal plans, assist with accessing services, and offer transportation as needed. Your flexible schedule will include evenings and weekends based on client requirements.
Key Responsibilities:
• Work with clients to assess needs and create intervention plans
• Develop support strategies addressing daily life challenges
• Engage clients in various environments to provide support
• Communicate effectively with professionals and family members
• Ensure the interests of clients are prioritized in services
Requirements:
• Diploma in Social Services with relevant experience preferred
• Experience supporting individuals facing multiple barriers
• Capability to handle crisis situations effectively
• Valid driver's license and well-maintained vehicle
• Successful completion of background checks required
