Home Health Social Worker Position

• Conduct psychosocial assessments and care planning
• Provide ongoing support and monitoring for clients
• Collaborate with various health care professionals
• Advocate for clients and connect them with community services
• Lead educational sessions for clients and caregivers

• Bachelor’s Degree in Social Work
• At least three years' experience in community settings
• BC College of Social Workers registration
• Valid BC Driver’s license required
• Robust understanding of clinical practice and community resources
