Community Outreach Worker

Position: Community Outreach Worker at Connective
Make a difference as a Community Outreach Worker with Connective, a leader in social services. This role focuses on client support and goal planning within the community.

In this person-centered position, you will provide outreach assistance to clients referred by Community Living British Columbia. Key tasks involve collaborating with clients to develop individualized goal plans, addressing their needs in housing, employment, and life skills. Flexibility is essential, as you'll work varied hours that may include evenings and weekends to ensure community support.

Key Responsibilities:

• Assess clients' needs and intervention possibilities

• Develop personalized intervention and support plans

• Address daily living issues for clients

• Liaise with professionals and families on client care

• Transport clients using your own vehicle

Requirements:

• Diploma in Social Services preferred

• One year of relevant experience is an asset

• Experience with vulnerable populations

• Valid driver’s license and personal transportation

• Able to pass a Vulnerable Persons Criminal Records Check
