As an Outreach Support Worker, you will empower clients referred by Community Living British Columbia, helping them adapt to community life.

Your role includes developing individualized plans that incorporate essential services and life skills while working during flexible hours to serve clients' needs effectively, including evenings and weekends.

Key Responsibilities:

• Evaluate client needs and potential interventions

• Create tailored support and intervention plans

• Facilitate access to housing, nutrition, and employment

• Connect with families and professionals for better care

• Use your own vehicle for client transportation

Requirements:

• Diploma in Social Services is preferred

• One year of experience in a relevant field

• Experience with marginalized populations

• Valid driver’s license required

• Must complete a Vulnerable Persons Criminal Records Check
