Salary: CAD $51.37 - $64.19 / hour (Full‑time)

Job Summary
We have a full‑time opportunity for a Clinical Pharmacist to provide pharmacotherapy services to community dwelling clients in Abbotsford, BC. Join our integrated team of four premier health employers.

• Consult with physicians, nurses, and other health care providers to develop therapeutic plans for each client.
• Individualize client medication therapy by reviewing clinical data and modifying plans as needed.
• Provide medication counselling to clients and families, demonstrating techniques and assessing understanding.
• Assist the health care team in developing drug therapy guidelines and monitor guideline effectiveness.
• Participate in client home visits, program team rounds, and meeting participation as needed.
• Act as a program resource person, providing detailed drug information and supporting staff.
• Provide academic detailing services to physicians to optimize prescribing practices.
• Contribute case studies and review articles to professional journals.
• Provide instruction and/or supervision to pharmacy students and residents.
• Participate in quality improvement activities and medication safety initiatives.
• Manage medication inventories and report procedural errors.
• Prepare reports on various activities and maintain electronic records.
• Perform other related duties as assigned.

• Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
• Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.
• Valid BC Driver’s Licence and access to a personal vehicle for business purposes.

• Broad knowledge of organization policies, procedures and standards of care.
• Knowledge of evidence‑based clinical pharmacy practice, medication use evaluation, research processes and methodology.
• Ability to apply clinical pharmacy knowledge and make decisions with minimal supervision.
• Strong communication skills, verbal and written.
• Capacity to establish and maintain effective working relationships with health practitioners, patients, families and the public.
• Organizational skills and ability to prioritize work.
• Ability to operate related equipment and perform required duties.
