About the role

Do you have excellent communication skills, an ability to work in partnership with others, a patient‑focused approach and a desire to improve services to meet patient needs? Then come and join our community dietetic team in Aberdeen City.

This postholder will work with frail patients (or those at higher risk of frailty) in the community, providing group or one‑to‑one education sessions to support people to improve their diet and health.

There is an exciting opportunity to be involved in a project to help identify individuals at risk of falls and frailty within general practice or local community settings, and to develop and deliver suitable education programmes to meet their needs.

The post will be supported by a Health Care Support Worker and dietitians already working in this area.

We are actively engaged in the use of technology to deliver care wherever possible and there will be opportunities to develop this further.

There is a high level of clinical administrative tasks involved in this post, from entering information on electronic patient records, to developing Excel spreadsheets to collect and analyse data; therefore we require an individual to be confident and competent in using IT.

We have well‑established links with dietitians across all sectors in Grampian and work together to provide a range of CPD and development opportunities, particularly around clinical effectiveness, patient safety and patient centredness.

The postholder will participate in undergraduate student training and training to a range of key stakeholders across the partnership.

You must be a car driver, hold a full UK driving licence and have access to a car.

For AHP posts within Grampian, there is a requirement to provide services flexibly to meet the needs of service users. This will include providing services over weekends and extended hours when necessary.
