, you’ll be at the heart of our regulated service, responding to referrals from health professionals and providing practical and emotional support to vulnerable adults. You’ll help prevent hospital re-admissions and promote wellbeing by enabling individuals to regain confidence and independence. Could you be the person who brings kindness and stability when it’s needed most?

• Visiting service users in their homes to assess needs and create tailored support plans.
• Assisting with daily living activities such as bathing, dressing, and meal preparation.
• Liaising with health, housing, and social care professionals to ensure consistent support.
• Carrying out welfare checks and helping individuals rebuild their independence.

• Experience supporting people with physical, sensory, or mental health needs.
• Strong communication skills and the ability to work both independently and in a team.
• Knowledge of NHS and social care services, and how to access them.
• A flexible approach to working hours, including evenings and weekends.
