At Rubislaw Place Medical Group, we operate a clinician-led triage model to ensure patients receive care from the most appropriate healthcare professional in a timely manner. All requests for same-day medical advice are assessed through our triage system, with clinicians reviewing the information provided and determining the most suitable outcome. This may include a telephone consultation, face-to-face appointment, referral to another member of the multidisciplinary team, signposting to an alternative service, or the provision of self-management advice where appropriate.

The triage process helps us prioritise urgent clinical need, manage demand safely, and make best use of the skills available across our clinical team, ensuring patients receive the right care, at the right time, from the right clinician.

An exciting opportunity has arisen for a motivated and compassionate Mental Health Nurse Practitioner to join our multidisciplinary team at Rubislaw Place Medical Group LLP.

This is a newly developed role within the practice, providing an excellent opportunity to work closely with our GP triage team to support patients experiencing mental health difficulties. The successful candidate will play a key role in improving access to mental health support within primary care, helping patients receive the right care at the right time.

Working alongside our GPs and wider clinical team, you will provide assessment, support, signposting, and interventions for patients presenting with a wide range of mental health concerns.

• Work alongside GP triage clinicians to assess patients presenting with mental health concerns.
• Undertake holistic mental health assessments and risk assessments.
• Provide support and advice for patients experiencing anxiety, depression, stress, bereavement and other mental health difficulties.
• Develop and implement care plans in collaboration with patients and healthcare professionals.
• Signpost and refer patients appropriately to secondary care, community services and third-sector organisations.
• Medication reviews of patients with chronic mental health issues.
• Provide crisis support and ensure appropriate escalation where required.
• Work collaboratively within the multidisciplinary team to improve patient outcomes.
• Maintain accurate clinical records and documentation.
• Contribute to service development and quality improvement initiatives.

• Be a Registered Mental Health Nurse (RMN) with current NMC registration.
• Have experience working within mental health services.
• Demonstrate excellent communication and interpersonal skills.
• Be confident assessing and supporting patients with complex mental health needs.
• Have experience of risk assessment and care planning.
• Be able to work independently whilst contributing effectively within a multidisciplinary team.
• Experience with in primary care, community mental health services or crisis services would be advantageous, although not essential.
