Community Care Worker - FLK

Job Description

An exciting opportunity has arisen within Falkirk Council for qualified Social Workers to join our teams across Adult Social Work Services, including our Locality Assessment and Care Planning Teams and the Care Home Assurance and Review Team (CHART).

As part of our vision to enable adults to live full, positive and independent lives within supportive and inclusive communities, you will work collaboratively with individuals, carers, care providers and partner agencies to undertake strengths-based, outcomes-focused assessments, reviews and support planning.

The successful candidates will contribute to a range of adult social work functions including assessment and care management, Adult Support and Protection, risk assessment and review, care planning, and supporting individuals living both within the community and in care home settings. Working closely with health colleagues, care providers, independent advocacy and third-sector partners, you will play a key role in promoting positive outcomes for Falkirk citizens.

You will participate in duty systems and rotas relevant to your team and contribute to the ongoing development of innovative and person-centred approaches to service delivery. Alongside colleagues, you will support continuous improvement and help ensure adults have access to the information, support and services they need to achieve their personal outcomes.

The successful candidate will be subject to a PVG Scheme check for Adults and Children.

You must register with the SSSC as a Practitioner in care home services for adults within 6 months and meet the conditions of your registration within three years.
