Overview
CASE MANAGER for Olympic Area Agency on Aging (O3A) based in Aberdeen. Provides case management for in-home care to seniors and adults with disabilities, including assessment of care needs, creating service plans, coordinating care providers, and focusing on client independence.
Responsibilities- Provide case management for in-home care to seniors and adults with disabilities.
- Assess care needs, create service plans, coordinate care providers, and promote client independence.
- Master’s Degree in behavioral or health sciences with 1 year of paid social service experience; or BA in a similar field with 2 years of paid social service experience; or BA with 4 years of paid social service experience.
- Requires current WDL and insured vehicle.
- Hours:
40 hrs/week. Non-Exempt. - Salary: $68,401 to $89,749 annually.
