Overview
ADRC Specialist provides accurate and relevant information to individuals seeking resources including older adults, family caregivers, adults with disabilities, and Medicaid/WA Cares beneficiaries. Operates with a significant level of independence and requires minimal supervision.
ResponsibilitiesConduct program and service eligibility screening and needs assessments. Deliver services and options counseling under targeted community-based programs, including Care Transitions, Health Related Social Needs (HRSN), Medicaid access, and WA Cares. Provide individualized and comprehensive short‑term case management services.
QualificationsRequires a Bachelor’s Degree in behavioral, health sciences, or related field with two years’ experience providing direct human services; or MA in behavioral, health sciences or related field with one year experience providing direct human service; or equivalent combination of education and experience. Requires current WDL & insured vehicle.
Compensation40 hrs./wk. Non‑Exempt. $59,646 to $69,375 annually.
