A local medical practice in Abergele is seeking a Care Navigator to serve as the first point of contact for patients, managing booking and administrative tasks within a supportive team. The successful candidate should possess strong interpersonal and communication skills, ideally with experience in a GP practice. Full training will be provided, but GCSE English and Maths are required.
This role offers 25 hours a week, with an emphasis on delivering excellent patient care and managing confidentiality.
