Functions as a liaison between home health and hospice agency and hospital to ensure coordination of care. Is part of the core services for the hospice interdisciplinary team (IDT/ IDG), this position will coordinate the plan of care focusing on quality, timely delivery of services, and medical necessity of a patient's admission, including what level of care is appropriate in coordination with the IDT/ IDG.
Assesses, plans, implements, monitors, and evaluates options and services to meet individual’s health care needs and the family systems that support the patient. Seeks positive health outcomes by ensuring well‑orchestrated, coordinated care focused on quality, efficiency and safety. Will deliver counseling and bereavement services and interventions to terminally‑ill patients of all ages and their families, caregivers or any other significant person(s) whom are facing significant loss or grief.
Provides grief and loss counseling services to residents of the community through community service relations. This position may require outside coordination through community relations within the community including after‑hours home visits, events and fundraisers. This position maintains an open, approachable manner treating others fairly and respectfully preserving self‑confidence and dignity as well as showing regard for their opinion.
- Master in Social Work
- One-year social work experience required
- Experience in a healthcare setting and/or dealing with families is preferred
- Experience with individual, couple, family dynamics and support group counseling preferred
- Experience in bereavement and grief counseling preferred
- Current Kansas LMSW license
- LSCSW license preferred
- Valid Kansas Driver's License with clear driving record
- Proof of vehicle insurance provided by insurance carrier
Schedule-Shift-Hours Full Time - Day Shift - Monday–Friday,8:00 AM to 4:30 PM
