New Horizons operated the only first two and longest standing Qualified Residential Treatment Programs (QRTPs) in the entire state—demonstrating our commitment to high-quality, trauma-informed residential care for youth with complex needs.

• FAYS (Family and Youth Success) – strengthening families and helping at‑risk youth
• Kinship Services – supporting relatives who step in to care for children in need
• Family Resource Center (FRC) – providing wraparound support and local access to resources for families in crisis or transition

At New Horizons, you won’t just be joining a team—you’ll be joining a movement to protect and uplift children and families across Texas.

• Collaborate with the Service Planning team to create and monitor individualized aftercare plans.
• Manage a caseload of approximately 25 youth receiving aftercare services.
• Conduct and participate in aftercare check‑ins to assess progress and needs.
• Assist with school enrollment and maintain educational portfolios.
• Coordinate medical and mental health service transitions to ensure no lapse in care.
• Document all services and outcomes accurately and timely in accordance with agency and state standards.
• Support parents and caregivers in accessing community resources and normative, age‑appropriate activities.
• Adhere to all New Horizons policies, state, and federal child care regulations.
• Maintain a commitment to New Horizons’ philosophy of Keeping Kids First in all actions and decisions.

• Bachelor’s degree and two years of experience working with youth who have experienced trauma preferred; or a master’s degree in a social service or related field.
• Valid driver’s license, proof of insurance, and ability to transport youth as needed.
• Strong organizational, communication, and documentation skills.
• Ability to build positive relationships and work collaboratively across disciplines.
• Understanding cultural diversity and the ability to work effectively with diverse populations.
• Must pass a criminal background check, drug screen, and tuberculosis screening.

$38,000.00 – based on experience, educational background, and specialized skills.
