Plans and coordinates intake and referrals for Hendrick Home Health, ensuring the timely receipt and review of accurate patient information, physician orders, and supporting documentation to facilitate appropriate admissions and quality patient care.
Job RequirementsMinimum Education
- LVN
- 1 to 3 years of experience as an LVN, preferably with experience in home health.
- CPR (BLS)
- Strong communication skills, verbal and written alike.
- Understanding of physician orders and clinical definitions.
- Knowledge of patient retention, ability to differentiate levels of urgency, and organizational skills.
- Knowledge of regulatory standards regarding physician offices and home health, specifically in compliance with Medicare.
