Transition Specialist
BCFS Health & Human Services is seeking a Transition Specialist who will use professional judgement and best practices to ensure quality case management services for youth transitioning out of foster care.
EducationRequired – Bachelor’s degree in Human Services, Social Work, Education, or a related Social Science field.
ExperienceRequired – 2 years of experience in case management, job development, or crisis intervention with youth, and experience in community building, event coordination, and public speaking.

Essential Functions- Assess personal needs of PAL clients and provide case management resources.
- Advocate with human services, health, and court and criminal justice systems to resolve client issues.
- Assist youth in crisis by providing de‑escalation and developing a plan to resolve out‑of‑state crises.
- Develop detailed Individual Service Plans, meet regularly to assess progress and update plans.
- Maintain timely and accurate file documentation and data entry per PAL program requirements.
- Follow all HHS guidelines and policies concerning PAL TFSS services.
- Demonstrate working knowledge of contracts and agency policies and procedures.
- Meet all federal and state regulatory guidelines applicable to this position.
- Participate/lead PAL program weekly, monthly events and activities.
- Work evenings or weekends as needed or requested by Program Director.
- Implement BCFS HHS safety protocols, including evacuating with children and staff during emergencies.
- Get approval for services via authorization form (2054) from HHS PAL staff.
- Show knowledge of community agencies and resources.
- Maintain confidentiality across all program operations.
- Maintain BCFS HHS professional and ethical standards of conduct as outlined in the employee handbook.
- Attend all required program trainings as applicable.
- Record all services and activities accurately, concisely, and timely using program forms and Salesforce database.
- Report suspected child abuse and neglect to Program Director and law enforcement.
- Maintain confidentiality with clients and staff.
- Respond sensitively and competently to the service population’s cultural and socio-economic characteristics.
- Communicate effectively in writing and verbally in English.
- Work in a fast‑paced environment.
- Maintain computer literacy required for the position.
- Work independently and as part of a multidisciplinary team with minimal supervision.
- Travel as needed.
- Other duties assigned per grant requirements.
- Maintain certificates required for the position, including First Aid and CPR.
- Have a valid Texas Driver’s license with a clear driving record and reliable transportation.
- Pass a pre‑employment drug screen and random drug screens.
- Provide proof of work eligibility status upon request.
- Pass a pre‑employment and biennial criminal background and motor vehicle check (fingerprints required if lived out of state within past 5 years).
