Triage Nurse - District Nursing

Our District Nurse Duty Desk provides a vital triage service, enabling patients to receive holistic assessments of their care needs and ensuring they are signposted to the most appropriate services. The team plays a key role in coordinating timely visits from District Nurse assessors, delivering responsive, patient-centred care within the community.

Oxford Health NHS Foundation Trust provides physical, mental health and social care for people of all ages across Oxfordshire, Buckinghamshire, Swindon, Wiltshire, Bath and North East Somerset.

The services currently operate 7 days a week, 365 days a year, with core hours of 08:00-18:30. Shift patterns are 08:00-16:00, 10:30-18:30 or 08:00-18:30.
