Job title: Housing & Resettlement Worker (Vineyard)
Role summary: Join us at The Vineyard in Abingdon, a supported housing project empowering people who have experienced homelessness to build independent futures. You will provide safe, high-quality, trauma-informed support, leading on case management for residents with complex needs. Working collaboratively with health and homelessness partners, you will develop holistic support and risk plans, promote wellbeing, and enable positive move‑on. Main Purpose Of The JobThis role is responsible for delivering a safe and supportive service to all residents living at The Vineyard, Abingdon. The Vineyard is a group of flats providing accommodation to people who have experience homelessness and are working towards independent living. You will lead on the case management of residents within the project with a history of homelessness and other support needs.
You will maintain and develop partnerships within homelessness pathways to ensure better outcomes for our residents. This includes identifying and pro‑actively working towards appropriate move‑on from The Vineyard. You will have experience in working in a psychologically informed way and building relationships through a trauma‑informed approach. The post involves a significant amount of lone working.
- Ensure the effective delivery of high quality support and housing management according to Homeless Oxfordshire policies and procedures.
- Proactively and dynamically assess, monitor, manage and respond to risk.
- Create individual holistic support & risk management plans for clients with complex needs.
- Work in partnership with health agencies to ensure clients have access to appropriate healthcare as needed.
- Work collaboratively with residents to identify and work towards suitable move‑on encouraging clients to develop independent living skills.
- Report any safeguarding concerns without delay in line with procedures.
- Work alongside the LEAP (Learning, Empowerment and Participation) team to ensure clients have access to opportunities to be empowered, build their confidence and develop their skills.
- Be a supportive and effective part of the team, including contributing to the induction of new colleagues. Keep up to date and clear records and ensure information is shared with the team and external agencies as required.
- Maintain positive relationships with all stakeholders.
- Represent Homeless Oxfordshire at inter‑agency meetings, as required.
- Undertake any other duties that can be reasonably required to meet the operational needs of Homeless Oxfordshire.
- Comply with Homeless Oxfordshire’s policies and procedures.
- Contribute to a positive working environment in which equality and diversity are valued and staff are enabled to do their best.
- Contribute to the work of the broader team.
