Life Skills Coach/Case Manager - Young Adult Independent Living Program. Intercept Health is a family-owned agency serving various mental and behavioral health, substance use, and foster care needs in Virginia. We offer a $3,000 sign-on bonus. • Actively engage with young adults, ages 17-21, who are preparing for successful transition into the community.
• Serve as a positive role model for residents to observe and learn socially effective values, attitudes, and behaviors.
• Support residents in developing money management skills, obtaining employment, completing their education as applicable, learning daily living skills, and exploring career development opportunities.
• Contribute to the formulation and implementation of the resident transitional living plan and assist with life skills learning, transportation, and administrative duties.

• Bachelor's degree in any field plus two years of experience in providing case management services to children and families; OR a human services Bachelor's degree (Social Work, Psychology, Counseling, Special Education, etc.) with experience providing case management services to children/youth and their families preferred; OR a human services Master's degree (Social Work, Psychology, Counseling, Special Education, etc.) with a student placement providing case management services to children/youth and their families.
