Clinical Social Worker-UAEN

Responsibilities

• Patient Assessment: Evaluate patients’ mental, emotional, and social needs, including family dynamics, financial situation, and support systems.
• Care Planning: Develop and implement care or discharge plans in coordination with doctors, nurses, and other healthcare staff.
• Counseling & Emotional Support: Provide guidance to patients and families dealing with stress, trauma, chronic illness, or end-of-life situations.
• Discharge Coordination: Arrange post-hospital care such as rehabilitation, home healthcare, or placement in long-term care facilities.
• Resource Connection: Link patients with community resources, financial aid programs, housing services, or support groups.
• Advocacy: Ensure patients’ rights are respected and help them navigate healthcare systems and insurance processes.
• Crisis Intervention: Assist in emergency situations like abuse cases, mental health crises, or sudden illness.

• Bachelor’s or Master’s degree in Social Work (BSW/MSW)
• Professional licensing (varies by country or region)
• Experience in healthcare or clinical settings is often preferred
